Bacterial status of the periapical radiolucent areas of endodontically involved teeth

The purpose of this study was to investigate the presence or absence of viable bacteria in the periapical areas or endodontically involved teeth.showing radiographic periapical rarefact1on. Fifty cases were treated in the Endodontic clinic. Each of these teeth was subjected to a battery or cultures during the course of treatment. The samples were taken from both the root canals and the periapical regions. The periapical cultures were taken employing cannulas and stylets with the cannula preventing the stylet from encountering the tooth and enabling it to enter the periapical space without contamination. The cultures were placed into Bacto cooked meat media and Brewer's thioglycollate broth and incubated for 72 hours. Further investigation of the positive cultures was done in an attempt to discover the types of bacteria present. The results indicate that the teeth not contaminated by saliva, and asymptomatic, have starile periapical lesions. On the average, 32 per cent of all of the lesions studied had positive periapical cultures. The influence of oral contamination and tooth sensitivity, increase significantly the probability of finding bacteria within the periapical lesion. This study has shown why teeth that had been ffopen" or sensitive, seem to be more susceptible to endodontic flare-ups between appointments. It was concluded that further investigation of the periapical lesions should be performed with the aim of correlating the bacteriology with clinical problems.
