Abstract
Since capillary basement membrane hypertrophy is routinely observed in many cases of diabetes mellitus (82), the authors of this paper felt that microvascular examination via biopsy might be efficacious in diagnosing the disease. Studies of diabetic dermal capillaries have shown great variations in occurence and width of thickened basement membranes (53, 77). These findings tend to render skin biopsy unreliable as a diagnostic tool.
In a study by Quart, Stahl and Sorin (81), gingival biopsies were taken from diabetic, hypertensive and normal patients. Degenerative vascular changes seen in the gingival specimens correlated well with those seen in retinal examinations which are considered to be sensitive indicators of the generalized vascular state. The investigators felt the gingival biopsy might be used as an aid to diagnose diabetes and hypertension (81). Yet Merimee, Siperstein, Fineberg and McKusick have observed that only 41 percent of diabetics have demonstrable retinal pathology (71). This finding casts some doubt upon the usefulness of retinal examinations and also indirectly upon gingival biopsy as a reliable diagnostic method.
The purpose of this study is to determine if microvascular changes occur in oral muscles of diabetic and prediabetic patients.
If so, biopsy of the buccinator muscle may prove to be a diagnostic aid in determining the presence and severity of diabetes mellitus. A comparison of capillary basement membrane hypertrophy with findings in the diabetic’s medical history and periodontal health will be made.
