The systematic use of erythromycin to prophylactically control endodontic flare-ups and the anaerobic status of the exudate after cleaning and shaping

Abstract
The problem of managing teeth which have been left open for drainage is still controversial. Most articles deal with the proper method for cIosing infected teeth in an empirical manner, depending on the experiences of the author. The purpose of this experiment was to test the hypothesis that the prophylactic use of systemic erythromycin would help control the endodontic flare-up. One hundred fifty teeth were studied. One half of the patients were given either prophylactic antibiotics or a placebo in conjunction with the cleaning and shaping technique as taught in the Graduate Endodontic Department at Boston University while in the other cases, the operators relied solely on the cleaning and shaping procedure. There was only one flare-up in the group that had received the prophylactic antibiotics and only one flare-up in the group of cases that did not receive medication. The incidence of flare-ups then, was 2 in 150 cases, or 1.3%. On the basis of these one hundred fifty cases, it can be concluded that the low incidence of flare-ups is dependent solely on the meticulous cleaning and shaping of the root canal system and not on the administration of a systemic prophylactic antibiotic.
