Exploring the perspective of health professionals and patients on safety and readiness for discharge

Abstract
OBJECTIVE: Studies have shown that effective discharge planning is one of the key factors of patient care, influencing safety and outcomes following hospital stays. The perception and understanding of hospital discharge by health professionals and patients are important in developing effective discharge planning. This study aims to explore the challenges faced by health professionals and patients during the discharge process through targeted interviews. By engaging with both groups, the study seeks to understand the existing challenges related to the discharge process and gain insights into the issues at hand. Additionally, the analysis of interview transcripts will focus on uncovering barriers to effective communication that may hinder a smooth discharge experience. Ultimately, this research aims to contribute to improved discharge planning and enhance patient safety and satisfaction. METHODS: Focus group interviews were conducted with different healthcare professionals, many of whom were responsible for coordinating the discharge planning process in Boston Medical Center as well as patients who were receiving care in the hospital. The discussion covered current challenges faced by participants in hospital discharge from both the patient’s and health professional’s perspectives. RESULTS: Based on the interviews the three major themes were identified. The first being difficulties in retaining and understanding crucial information and possible solutions. Additionally, barriers and possible solutions to effective communication during discharge were a theme identified. Lastly, lack of accessibility to support systems & resources was identified as a theme. CONCLUSION: By addressing these challenges, healthcare providers can better support both patients and their families, ultimately leading to more effective discharge planning and improved health outcomes.
