Clinical and radiographic observations of the periodontium in patients undergoing orthodontic therapy: first fifteen months

Abstract
Twenty-two male patients from 12 - 14 years of age were selected prior to active orthodontic therapy at Boston University School of Graduate Dentistry's Department of Orthodontics. These patients were pro - jected to undergo full-banded orthodontic treatment. The objective of this investigation was designed as a long term project to document the quantitative and qualitative changes occurring in the periodontium during and following orthodontic therapy. The patients were studied for the first 6 month period by Dr. Stuart Sears and Dr. Sergio Tejedor-Leon. Relocation of two families reduced the number of patients to twenty which have now been examined at nine, twelve and fifteen months. The examination included a clinical examination, models, photographs and radiographs (at the 15 month interval only). The results were recorded on a Data Collection Form which allows efficient collection and computerization of the data. That which follows is an abstract of these results seen between Examination 1 and Examination 6: Sulcus depth both buccal and lingual, and the attached gingiva on the facial, showed a significant increase. There was no significant increase in the attached gingiva on the lingual. Although the sulcus depth increased in all patients, the magnitude of the increase in mesial and distal (since there was no difference between them) was significantly greater than the increase in mid-sulcus depth on the lingual and facial. The Plaque Index and the tissue inflammation index increased. The height of the gingival tissue increased both on the buccal and the lingual. There was no significant change in mucogingival problems on both the buccal and the lingual. When the variable was the difference between Examination 1 and Examination 6 there was a significant correlation between: a) Plaque Index vs. Inflammation b) Sulcus depth facial vs. sulcus depth lingual c) Attached gingiva facial vs. attached gingiva lingual d) Attached gingiva lingual vs. height of gingiva on the buccal. A significant relationship existed between the use of orthodontic appliances and 1) the development of tissue bunching and 2) an increase in the size of the adjacent interdental papilla. There was a significant decrease in the amount of actual attached gingiva both on the buccal and lingual.
