Build-up of endodontically treated teeth with bonded new ceramic systems

Abstract
This in vitro study evaluated the fracture resistance and light transmitting properties between maxillary incisors that had been restored with different post systems in combination with bonded full ceramic crowns and endodontically treated maxillary incisors without coronoradicular reinforcement. Three post and core systems were under investigation: one cast post and core, one zirconia post with a bonded glass-ceramic core, and one experimental resin-ceramic interpenetrating phase composite post (RCIPC) also in combination with a bonded glass-ceramic core. In the coronal portion, the samples restored with zirconia or RCIPC posts exhibited the greatest values of lightness and were significantly different from the other groups. In the radicular portion, differences in light transmitting properties could be shown with microphotographs under transillumination, but couldn’t be measured by spectrophotometrie. Each specimen was intermittently loaded and thermocycled before the final stress tests in a Zwick machine. Although some of the PROCERA (Nobelpharma) copings fractured in the oral artificial environment, they demonstrated excellent strength values for an all-ceramic system. Specimens without any coronoradicular reinforcement exhibited the highest fracture resistance, whereas the samples with RCIPC posts showed the lowest. Samples restored with a cast post and core or zirconia post were not statistical significantly different with respect to their failure load. Teeth restored with zirconia posts demonstrated the most favorable failure mode. It was concluded that the preservation of both internal and external tooth structure is of utmost importance when restoring endodontically treated teeth.
