Abstract
This template guides medical providers and trainees through a structured, 20-minute virtual interprofessional CGA team meeting, with a note outline organized by the Geriatrics 5Ms and input fields for nine health professions.
Description
Template for and illustration of the workflow for conducting a geriatric evaluation/comprehensive geriatric assessment (CGA) interprofessional team meeting. Includes learning and clinical objectives, a step-by-step workflow for implementing the meeting in an electronic health record (CPRS), and a structured note outline covering: active problems; synthesis of information; and a plan of care organized by the Geriatrics 5Ms (Mobility, Mind, Medications, Multicomplexity, Matters Most), discharge planning, and input from nursing, social work, nutrition, occupational therapy, physical therapy, pharmacy, psychology, recreational therapy, spiritual care, and speech therapy. Format: virtual (Microsoft Teams or equivalent); 20 minutes per patient; participants include approximately 9 interprofessional team members.
